Accident Medical Benefit*
This Accident Only Medical Expense benefit covers:

up to $10,000 (Platinum Membership)

up to $7,500 (Gold Membership)

up to $5,000 (Silver Membership)
 
up to  $2,500 (Bronze Membership)

for all necessary medical and dental expenses per occurrence that
are a direct result of an accidental injury.

With a $100 deductible, this plan covers active members and all family members under age 70.

The benefit also includes an AD&D benefit of:
$50,000 (Platinum Membership)
$25,000 (Gold Membership)
$10,000 (Silver Membership)
$7,500 (Bronze Membership).

$300 per day In-Hospital Accident Benefit*
This benefit provides for a $300 per day Accident In-Hospital Indemnity Benefit per insured, per occurrence up to 30 days per injury.

Exam Plus Vision Benefit from VSP**
The Exam Plus Plan will cover your eye exam in full every 12 months with a $15 co-pay. )

Roadside Assistance Program
Roadside Assistance is available 24 hours a day, 365 days a year to assist members when owned or leased vehicles are disabled as a result of unavoidable circumstances.

Includes Towing Assistance, Flat Tire Assistance, Fuel, Oil, Fluid and Water DeliveryService, Lock-out Assistance, Battery Assistance, Collision Assistance.
